Ok. The "PHONE" problem is that the radio thinks that a call is coming in. This is because:
1) the "tel mute" pin on the rear of the unit is being pulled to ground.
2) the unit is mis-reading the fault.
to determine which it is, disconnect the accessory connector from the rear of the unit. There are 3 multi-plugs on the back, one for power; one for speakers; one for accessories. it's the shorter, wider on in between the others. There may also be a green CD changer connector and there is the antenna.
If the fault is still present, then I'm afraid that it is a faulty radio. try cleaning the connector PCB with IPA or similar.
If the fault disappears, then it is most likely that a mobile phone kit has been installed and is causing the problem. usually, there will be a wire to the accessory connector that has been added. eg. not in the vehicle loom. This is usually yellow. simply remove / cut this wire. This should clear the fault.
Hope this helps.
As for the "VOLUME/SPEEDO" problem - these units have a feature called "AVC" - Automatic Volume control. The unit receives a signal from the PCM which tell it how fast the vehicle is travelling. the unit then adjusts the volume to compensate for road/engine/wind noise.
It is (in my opinion0 likely that your VSS - Vehicle Speed Sensor is faulty. I would recommend replacing this, as the PCM relies on this signal to manage the engine fuelling/ignition properly. Also, if you have a fuel/trip computer fitted, this will be giving false readings.
I hope I have added some useful info here.

If there has been at any time, such as mine had, a mobile phone kit fitted, as previous people have said there will be a yellow wire fitted.

I simply took the in line fuse out of the wire as I don't believe cars an phones mix.

That has solved my radio showing 'phone' all the time and my radio now works.
I appreciate the other input from your web site to allow me to corect this fault.

I've just encountered the same problem after entering the code.

I found this answer on another website - no idea yet if it will work:

Radios have a wire built in for future connection to phone kits for muting the radio. I think all that the phone kit does is give it an earth when a call comes in. So if this wire happens to earth on a part of your car whether a car kit is installed or not you will get phone showing on the radio. Pull out radio and find this unused wire. It is probably touching on something.

Mine has a similar problem where the radio mutes frequently and displays phone.

hy
check the connector circuit board inside the radio
you should find some rust or corosion that short-circuits the pins. clean with some alcohol, and that should do the job.

The 6000 in my Focus has just developed the same fault. 'Phone' is displayed permanently and none of the buttons work except the volume and Bass/Treble adjustment. Tried disconnecting the power with the hope that on re-powering and entering the security code that it might clear the problem. Wrong.

Phoned the dealer I bought the car from, and spoke to one of the mechanics who suspects that the phone/mute circuit board has given up, and that I need a replacement radio. As the car is still under warranty, I'm hoping the radio is as well. I know this is of no comfort if your unit is out of warranty.

I can't help much, but my RDS 5000 (same model but with acassette instead of CD) had always been temperamental for the 3 or more years that I had owned the car. It suddenly developed a fault where it would flash up PHONE for some time then go back to the radio. I never had a phone kit in it, the fault wasn't connected with any other events, so I think it's just the electronics going bonkers. And I think the RDS series has a bit of a reputation for that.

My solution was the slightly pathetic one of buying a 6000 to replace it on eBay for about £50.

My thoughts are that it might well not be fixable if its mad chips.
