how can I fix my transit gearbox?

I have a ford transit 2 litre turbo diesel, it is front wheel drive.I have a problem with the gear box, it crunches in to third, and jumps out of first and second (when first selecting but once in it's fine). I was just wondering if anyone knows weather it is the gearbox that needs changing or another fault?
