Sounds as though you are getting insufficient water flow across the exchanger. Try increasing the flow rate via the cold water supply tap located underneath the boiler. The other thing could be that the flow regulator valve situated on the DHW supply pipe right at the exchanger is swollen and reducing your water rate through the exchanger.
