What do I do about getting the glass on my top oven door fixed?

The inside glass panel on My 720 EF Heritage top oven door has exploded for no apparent reason.
Oven was on, door closed, and the glass blew with an almighty bang!
(a) Is there any history of this fault??
(b) What do I do about getting it fixed?
(c) Is the main oven door safe?
